There is high intensity bilaterally in the lentiform nuclei and left caudate head.  High intensity is also noticed in the substantia nigra.  The distribution of injury is in keeping with a metabolic condition.  Single voxel spectroscopy demonstrates an elevated lactate peak supporting the impression of mitochondrial disease as the underlying etiology.
