Can We Predict The Occurrence Of Hypsarrhythm In Term Newborn With HIE ?: A Prospective Observational Study

Perinatal hypoxic–ischemic encephalopathy (HIE) is an important cause for Infantile epileptic spasms(IS). but there is no much literature available regarding the markers for early identification of such infants based on EEG patterns before development of IS. Hence this study was planned.

All term neonates with moderate to severe HIE born from September 2022 to December 2023 were enrolled in the study. EEG was done at particular time periods - Within first 2 weeks, 1 month, 3 months, 6 months of age along with developmental assessment done till 9 months. EEG was reported by 2 Pediatric neurologists independently.

total 42 newborns were enrolled. Abnormal background was seen in 38%, 33%,30%, 36% at 2 weeks, 1 month, 3 months and 6 months recordings respectively with most of the infants having abnormal EEG background. 4 out of 5 infants having epileptiform discharges on EEG developed clinical seizures seen in 4. 2 out of 5 infants having discontinuous background at neonatal period with persistance at 1m and 3m of age developed IS with corresponding burst suppression variant of hypsarrythmia on EEG.

Abnormal EEG background was the commonest abnormality noted in 1/3rd of the neonates and many continued to have the abnormality in their successive EEGs. infants with persistent discontinuous background at 3months of age are at high risk of developing west sydrome.epileptic spasms.
