Electro-clinical Spectrum Of Children Having Epilepsy Secondary To Perinatal Asphyxia As Per ILAE 2022 Diagnostic System

Introduction: There is paucity of data on the electro-clinical spectrum of epilepsy in infants and children and the use of the ILAE 2022 diagnostic system in this age group. This study was planned to study the electro-clinical spectrum of epilepsy secondary to perinatal asphyxia as per the ILAE 2022 diagnostic system. Methods: This was a hospital based cross-sectional study. Infants and Children up to 18 years diagnosed with epilepsy with history of perinatal asphyxia were enrolled. Details of neonatal period, age at onset of epilepsy, types of seizures, EEG and MRI findings, ASM ,drug resistant epilepsy and co-morbidities were noted. Epilepsy syndrome diagnosis was done as per the ILAE 2022 diagnostic scheme. Results: 120 children were enrolled in the study. Infantile epileptic spasm syndrome was the most common epileptic syndrome, seen in 51 children (42.5%), followed by lennox-gastaut syndrome seen in 6 children,(5%). 58 (48.3%)children had non-syndromic epilepsy. 97 children (80.8%) had abnormal EEG, most common being hypsarrhythmia (39.1%) followed by multifocal discharges (29.1%). Most common MRI abnormality was atrophy(63.3%) followed by gliosis (40%) and encephalomalacia (38.3%). 100 children (83.3%) had developmental delay, 87 (72.5%) had cerebral palsy, 61 (50.8%) had vision impairment, 20% had hearing impairment, 66.6% had microcephaly as co-morbidities. 53 children (44.1%) were found to have drug refractory epilepsy. Conclusion: Infantile epileptic spasm syndrome was the most common epilepsy syndrome noted. Neurodvelopmental co-morbidities such a global developmental delay and cerebral palsy were common, indicating the need for screening and multidisciplinary care.
