The effect of anti-epileptic treatment selection on prognosis in traumatic brain injury

Introduction Traumatic brain injury(TBI) is a common cause of death, disability in children. A seizure seen in the first week after the trauma is called an early seizure, a seizure after the first week is considered a late seizure. Post-traumatic epilepsy(PTE) is the occurrence of one or more late seizures after TBI. In our study, we aimed to evaluate the characteristics, the choice of antiepileptic treatment, its efficacy, and its effect on prognosis in patients who presented with TBI and had seizures. Method Our study included 126 patients aged 1 month to 18 years, who presented with seizures after traumatic brain injury and were followed up for at least 2 years between 2015 and 2020. Retrospective records were reviewed. Non-traumatic seizure causes were excluded from the study. Trauma etiology, Glasgow coma score at admission, imaging findings, antiepileptic selection, EEG findings were examined. Findings The findings of the patients according to the seizure time are summarized in Table.1. In our study, it was seen that only the time of seizure had an effect on morbidity. (p=0.001) Levetiracetam was found to be as effective as Phenytoin in preventing seizure recurrence. (P=0.003) It was observed that the choice of antiepileptic didn’t affect the development of PTE.(p=0.803) Result Acute symptomatic seizure time is one of the important reasons for the development of epilepsy in children with TBI. Posttraumatic antiepileptics are effective in preventing seizures and relapses. It was found to be similar to the literature that it didn’t prevent PTE.
