CLINICAL PROFILE OF HEADACHE AMONG CHILDREN ATTENDING EMERGENCY DEPARTMENT AT ALEXANDRIA UNIVERSITY CHILDREN'S HOSPITAL

OBJECTIVES This study aimed to estimate the magnitude of headache among children attending the emergency department (ED) and to study the clinical profile, classification, etiology, laboratory and radiological investigations, and demographic characteristics of studied children with headache. METHODS A Descriptive observational study conducted at the Emergency Department included all children presented to ED with headache as their primary complaint over six months duration through semi-structured sheet that was designed, and implemented to the studied children. RESULTS Total headache cases represented 0.72% of total ER cases (n = 164/22662). 61% of total headache cases were having primary headache and 39% were secondary headache. Total admitted headache cases were 1.17% of total admitted cases (n = 46/3920). Prevalence of migraine among total headache cases was 51.8 and tension type headache was 9.1% table1. There was no statistical significant difference between studied children with primary and secondary headache regarding the sex, but there was statistical significant difference according to the age as secondary headache is more represented in preschool children, while the incidence of primary headache increases with age till become almost equal in adolescence. Primary headache children fulfilled the ICDH-3 criteria, while secondary headache children usually have red flags in their history and/or abnormal examination. Neuroimaging was not needed in 51.8% of children, considered in 18.3% of children, needed in 29.9% of children CONCLUSIONS Migraine is the most prevalent type of headache in children presented to (ED). Secondary headache is more common in preschool children while primary headache incidence increases with age.
